To get the sliding scale rate, you can also establish an appointment with a patient service representative to see if you qualify for a sliding-scale fee discount. You’ll need to provide your tax forms, last 30 days of check stubs and/or a letter from your employer. Sliding Fee patients will have a minimum co-pay of $20 for medical services. For dental services, the minimum co-pay is $25 for preventative services or $50 for restorative services.
